You don't understand earlier in the mass effect series you killed the last of the rachni as in the last one and yet they add rachni back because they needed more mobs. And there are a few of these such as you can have legion die in me2 but hell still be alive in me3 not just blocking out that part of the story. too little too late

You could have killed the last rachni queen. That doesn't mean that there couldn't have been some crazy ass rachni like in the hot labs scattered around the galaxy. As for legion he was a synthetic. A special synthetic that he housed not 100 geth programs like normal troopers but 1,183 geth programs which allowed him to be what he was. So whos to say someone didn't find him and bring him back to life?
